An investigation by University of Manchester researchers has pinpointed which prescription painkillers pose the greatest threat of fatal overdose.

The study identified fentanyl as the most hazardous opioid, with patients prescribed the synthetic drug facing triple the likelihood of experiencing dangerous breathing complications compared to those receiving morphine.


Fentanyl users were also found to be 85 per cent more susceptible to respiratory depression than morphine patients.

The powerful synthetic painkiller, roughly 100 times stronger than morphine, has been linked to 8.6 million cases of misuse in the United States alone.

Respiratory depression occurs when breathing becomes dangerously slow or shallow, causing carbon dioxide to accumulate in the bloodstream and potentially triggering cardiac arrest or respiratory failure.

To reach their conclusions, the Manchester team examined electronic health records from 32,909 adults receiving treatment at hospitals across north-west England.

Scientists specifically monitored patients whose vital signs indicated their breathing had dropped to perilously low levels following opioid administration.



The research team's access to detailed hospital records allowed them to track precisely when medications were given to patients and observe subsequent changes in respiratory function.

Beyond fentanyl, the investigation revealed that oxycodone and morphine both carried elevated risks of breathing difficulties when measured against codeine.

Codeine emerged as the safest option among the opioids examined by the research team.

Perhaps most concerning was the finding regarding polypharmacy.

Patients taking more than one opioid simultaneously faced a 50 per cent greater chance of respiratory complications compared to those using morphine alone.

The data underscores the compounding dangers when multiple powerful painkillers are combined, a practice that significantly amplifies the threat of life-threatening breathing problems.



Dr Meghna Jani, senior clinical lecturer at the University of Manchester and the study's senior author, stressed that these medications remain essential for treating severe acute pain.

"Opioids remain important medicines for managing severe acute pain. Our findings show that the risks are not the same across all opioid drugs or doses," she stated.
