US District Judge Sherilyn Peace Garnett delivered the sentence in Los Angeles federal court, matching what prosecutors had requested.
Mr Iwamasa, 60, had pleaded guilty in 2024 to conspiracy to distribute ketamine causing death.
He acknowledged administering the dissociative anaesthetic to Mr Perry via injection three times on 28 October 2023, the day the actor died.
